ARISE - Diabetes Self Management Program ARISE offers the Diabetes Self-Management Program, a series of six peer-led workshops that can help you gain the self-confidence and skills needed to manage diabetes. The program is highly supportive and is designed to complement or enhance the health care you may already be receiving. It is open to people with type 2 diabetes. CITY PARKS AND WALKING TRAILS Nine of the fourteen city parks have playground equipment. Wooded walking trails available behind G. Ray Bodley High School and Bullhead Point Park. North Bay Campground has trails, but they are closed from May 1 to Sept. 30 during the camping season. All other trails are available all year long. Recreation

HUD OFFICE Rental Assistance Program The City of Oswego Rental Assistance Program is a federally funded program that assists eligible low-income families, the elderly, disabled and handicapped pay a portion of their monthly rental payments based on household income and family composition. Housing
KINDRED AT HOME Kindred at Home (formerly Gentiva Home Health) specializes in home health care visits and is a fully certified and licensed agency by NYS Health Department in Oswego, Onondaga and Cayuga counties. Payment is accepted from Medicare, Medicaid, insurance and private pay. All levels of care are provided utilizing Registered Nurses, Licensed Practical Nurses, and Home Health Aides. The comprehensive home care program utilizes therapies such as Physical, Occupational, Speech, Medical Social Work and Nutrition. LIBERTY RESOURCES MOBILE CRISIS- ADULTS & YOUTH The Liberty Resources Mobile Crisis Team responds to individuals in crisis, over the phone or in person, in order to reduce emergency room visits and ensure everyone receives proper care. Our team of professionals are highly trained in mental health crisis management and suicide prevention. Mobile crisis teams can provide mental health engagement, intervention and follow-up support to help overcome resistance to treatment. The teams may offer a range of services, including: assessment, crisis intervention, supportive counseling, information and referrals to community-based mental health services and other support services. LIFESTYLES CENTER - SUNY Oswego SERVICES: The Center is a comprehensive and proactive prevention program that works toward providing a campus environment which promotes low-risk choices for students and reflects the attitudes, behaviors and values of healthy lifestyles. Peer Educators are trained to facilitate on-campus as well as off-campus presentations that deal with alcohol and other drugs, general wellness, violence prevention and Sexual Health (including abstinence) Certified Peer Education (CPE) training is provided. NATIONAL RUNAWAY SAFELINE Serves runaway and homeless youth in crisis throughout the United States by providing non-judgmental, non-directive crisis intervention. NEW YORK STATE ADDICTION HOPELINE This helpline provides 24-hour customer-focused addiction crisis counseling and statewide referrals to services for problem gambling, alcoholism and substance abuse, as well as follow-up calls 48 hours later for those who wish to be contacted. Addiction Services
NEW YORK STATE DEPARTMENT OF LABOR The New York State Department of Labor provides extensive and cost free employment-related services for both jobseekers and employers, including access to our universal employment database, staff recruitment coordination, applicant screening and job referral, employment/career counseling, jobseeker workshops and one-on-one re-employment assistance. OSWEGO COUNTY HOUSING ASSISTANCE PROGRAM Funded by the U.S. Department of Housing and Urban Development, this program provides Housing Choice Voucher (HCV – formerly Section 8) rental assistance to tenants of very low income households. Eligible individuals and families pay maximum of 40% of their adjusted gross income for rent and utilities. Program pays difference between tenant's payment and total costs of rent and utilities.
